Your confidentiality is a priority at the HEDCO Clinic. We are committed to upholding all legal and professional obligations to protect the confidentiality of your health records. Confidentiality is central to the effective provisions of healthcare services. A confidentiality medical record is established and maintained for every patient who receives care at the HEDCO Clinic.

Please see our Notice of Privacy Practices (PDF) for a complete description of how your health records may be used or disclosed, and how you can get access to your records. To obtain a copy of your medical records, you will need to complete a Release of Information form. You may also come into our office and fill out the form in person if you have any issues with the online method.

Assistance with this form is available Monday through Friday from 9:00 a.m. to 5:00 p.m. Please allow up to 10 business days for the processing of your request.

If you are an outside clinic requesting documentation, your client will need to have a Release of Information on file with us. Please send any requests via FAX or confidential email.
